The face of a serious med error and the consequences – watch video !

This 35 y/o young man was dispensed/provided not the medication for his existing medication condition as diagnosed and prescribed by this his doctor…but wrongly dispensed a chemo agent… whose normal dose is EVERY SIX WEEKS for FOUR DOSES, but ended up taking this wrong medication daily for 20 days before the error was uncovered.

You need to specifically watch the last couple of minutes of the 18 minute video on the funeral home website.. and see how this “med error” caused his very being to deteriorate over about 6 months and finally taking his live… while none of us know how long we will live.. given a average life expectancy.. Joshua.. should have had ANOTHER FORTY YEARS – or more to live.

Is this just another collateral damage of ever increasing staffing hours cut… while volume increases… every increasing metrics that have to be met … the hostile work environment that exists today in the typical chain store.. or the constant interference and micro managing of upper management.

Maybe this is just an example of how the “business of pharmacy” has become more important than the “practice of pharmacy”.

  1. Very sad but this one-sided story is missing a lot of details… Is there anything published about the event besides the the funeral home video? Not denying it happened but would be interested to know then pharmacy end of it. Was the script illegible? Maybe patient was in a rush and refused consultation? Look-alike, sound-alike drug? So many factors could be involved here. Not really fair to pass judgement without the full story.

    • Did a little more research and still the story has holes. But Regardless, very sad that this young man died so soon. Pharmacy has become like a fast food establishment. Unfortunately these trashes will likely continue because no one wants to wait, hours keep getting cut, and the head honchos only care about metrics,

      • Interesting that you didn’t mention what other things you found out. Sliding facts under the rug just like politicians. When a pharmacy is so blatantly making errors something needs to be done stat…to the whole pharmacy system, throw out those metrics! I love the business of pharmacy but it’s gone to far from caring for the patients

        • I didn’t mention what else I found because my point was that the article on THIS website was one-sided and missing information. The drug prescribed was lamotrigine; lomustine was dispensed in error. Important details to include if one is trying to determine root cause.
